Community Nurse Specialist Team

Many St Peter's patients receive our care from the comfort of their own homes.

Our Community Nurse Specialist team can support you at different stages of your illness to talk about what is most important to you. They can provide you with expert advice on symptoms and medication, as well as social and emotional support. Crucially, they work alongside GPs, hospital teams and district nurses to make sure that all the care you receive is joined up.

After you are referred to us, our nurses will arrange an initial assessment, where they will talk through how best they can support you. They will check your symptoms and ask you how you are feeling. If it’s helpful, they can give you and your family advice about making decisions about the future.

We've updated our look

We want everyone to get the care they deserve, which is why we're aiming to double the number of people we support by 2030.

To achieve this, we're delighted to introduce you to our updated look. Research carried out last year revealed there were many barriers to people accessing our support. Our modernised look helps breaks down misconceptions about St Peter's, which will help us connect with more of our local communities.
